Patient Education Materials

These were previously sold as pads, but are now available as a downloadable PDF at no cost! All illustrations are in one file, each on a separate page. These forms are useful for general patient education and informed consent. The podiatrist can use the basic anatomic features depicted on each form to further illustrate the particular deformity and the procedure to be performed.

There are seven different illustrations: Hallux Valgus, Heel Cord Stretch, Ingrown Nail, Digital & Metatarsal Deformities, The Normal Foot, Dorsal & Plantar View, and Rearfoot. Each pad is sized 8 1/2 x 11.
